CHS underserved status identification  (United States)

For both CE, CBE, and community health systems, we need a component where we can enter all the facilities and providers that we work with and identify/define their underserved status (CHC, MHC, IHS, etc).  This information would automatically populate our tables anytime that facility or provider is listed.  We've found that with multiple people collecting data from various sources, that sometimes we may not all be calling apples apples. 

A feature that allows  us to define the status and then automatically populates the entry would helpd standardize our tracking and make reporting much easier for our coordinators.
