To help out the Avian Flu Resource/Module Team please post any resources for avian flu prevention.  Deadline - Friday, Oct. 28, 2005 - 5 PM

Please provide the following information for each one.

Title (name of resource):

Media Format (Type: powerpoint, poster, PSA, etc.)

Description (describe the resource):

Objectives (state the objectives, if any):

Topics Addressed (List key topics addressed):

Audience (Identify the audience material is suitable for):

Availability (describe how to obtain materials (weblinks, cost, etc.)

Source/AHEC (AHEC staff member who located the resource and which AHEC they are employed)

Please follow the headings and examples closely to come up with your responses; I have included a sample to help you out. Please let me know if you have any questions.

I am currently unable to post the word document we are using to collect the data, but will do so as soon as that feature is fixed.  Otherwise, just post your results as a reply to this thread.
