DFW AHEC Quality Assurance Report

Findings:
- Some coordinators were not putting the ending date on activities, or put the wrong ending date.
- The duplication of school names/programs needs to be corrected as it caused some issues.
- There was confusion about how to use the “program” field on the Activity Form.
- Some important partners were omitted from activities.
Other comments unrelated to data input:
- Is there a way to link participant forms (G100) with resident/student forms (B100)? The reason I ask is because we have two cases where students from Camp Med are now involved with B100 type activities. It doesn’t apply this year as the participants were in Camp Med in the summer of 2005 before we started recording things in iahec. However, I see this as a great way to track our students!!!
- HCP Matriculation-Tracking form shows the number of enrichment program completers. To me that is not really matriculation and tracking.
- On custom reports you cannot search participants by date.
- What is DSCPH2 Type of site? Why don’t any of our sites show?
- What is “students into health careers diversity”? Is this enrichment program participants by diversity?
- What is CBE Other Activity by Trainee? We have a LOT of activities for health profession students that are not rotations. Shouldn’t they show here?
- There is a report for HCP Health Careers Events. Where is that info pulled from? If it is from the “program” field on the activity form, that is a problem because we are not using that consistently.
- Why are we not able to get a report for Health Literacy activties?
